Statement Date: The date that the bill was createdAG requests that you pay your bill in full upon receipt unless otherwise specified.
Chart Number: A special reference number for you that AG usesAll of your visits to AG will be tracked by your chart number.
Services Listed: The services performed, by codeOnly the services that AG and/or Austin Endoscopy Center provided to you will be listed on your statement of accounts.
The offices of your (non-AG) physician/anesthesiologist, pathologist, laboratory, and/or hospital will be responsible for billing you for the services they provided.
You might receive two separate bills if you had a procedure done: one from AG for your office visit and one from Austin Endoscopy Center for the procedure.
