The Valley Medical Center, Ambulatory and Community Health Services, Mental Health, Alcohol/Drug Services, and Public Health are the five departments that make up the Santa Clara Valley Health and Hospital System (SCVHHS). The primary provider of outpatient services and home for the Valley Healthcare for the Homeless Program (VHHP) is Ambulatory and Community Health Services (ACHS).
In addition to a shelter clinic, two mobile medical units, and two mobile dental units, ACHS runs a network of ten fixed sites. 11 access points for health care are formed by nine of ACHS’s fixed sites, a mobile medical unit, a mobile dental unit, and people who are homeless or at-risk of becoming homeless.
Dental, ancillary, laboratory, radiology, pharmacy, health education, care management, nutrition, and social services are all offered by ACHS in addition to primary and specialty care. The goal of the service strategy is to establish a “medical home” using a care coordination model to increase access to healthcare, improve health services, foster continuity of care, and fortify ties to non-medical housing and support initiatives.

In order to provide shelter, housing, food, employment, faith-based opportunities, and other support services to homeless people, ACHS collaborates with its sister organizations in SCVHHS, other county departments, the Emergency Housing Consortium, Gardner Family Health Network (also an HCH grantee), the Community Health Partnership, a group of community clinics, the Children’s Health Initiative, and a variety of other organizations. Additionally, ACHS works in partnership with the Santa Clara County Collaborative on Housing and Homeless Issues to provide homeless people with health care, housing, basic necessities, and other services.
